How to use it

  1. Use one row per state where an employee actually works or is expected to work.
  2. Verify state requirements with the responsible workers' compensation authority and insurer before treating other-states wording as sufficient.
  3. For monopolistic-state exposure, ask specifically how employers' liability is addressed instead of assuming the state-fund policy includes it.

What this template cannot prove

A worksheet or email template cannot create insurance coverage, establish legal worker status, modify an insurance policy, or guarantee that a contract requirement has been satisfied. When a field depends on coverage, confirm the policy or endorsement. When it depends on state law, use the responsible state authority.
